Oak Farm students can participate in sports starting in the 5th grade through the 8th grade level. All sports offered are co-ed.
All athletic forms are good for ONE calendar year. So if your son/daughter has already played a sport at Oak Farm Montessori this calendar year, and has his/her paperwork on file, you do not need to re-submit them, you will just need to submit the online form. *An athlete/parent handbook signature form must be signed for each participant.
Unless each child/participant has a current athletic form on file, all pages of the IHSAA form need to be printed, signed and turned into the Athletic Director prior to start of practice.
